At this time, Dr. Ashleigh does not offer OMT for children <12 years old
Dr. Ashleigh is an out-of-network provider. This means that there is no billing of insurance and payment is due in full at the time of service. This was a conscious decision on Dr. Ashleigh’s part to allow herself more time to spend with her patients as well as to remove the anxiety associated with surprise medical bills for those she cares for.
The office will provide you with a superbill (medical receipt) that can then be submitted to the insurance company for reimbursement. If you would like to inquire with your insurance company about their rate of reimbursement before your treatment, the medical billing codes typically used are 99205 for a new patient office visit and 99214 or 99215 for follow-up office visits.
We accept cash, personal checks, and credit cards as valid forms of payment. Patients can also utilize their Health Savings Account (HSA) and Flexible Spending Accounts (FSA) if they have them. When you schedule online, you will be asked to enter your credit card information to hold your appointment, but you will not be charged until after your visit. You are free to pay with whichever method you choose at that time. Please make checks payable to Integrative Medicine of Pueblo. A $25 service charge will be assessed to your account for all returned checks.
We are happy to accept patients with Medicare insurance, but the cost of a visit is legally required to be paid fully out-of-pocket and cannot be billed to Medicare. Therefore, at the initial visit, patients with Medicare will be required to sign a release acknowledging this policy. You can learn more about this policy by reading the Opt-out providers section at the webpage linked here.
Unfortunately, we are legally prohibited from seeing patients with Medicaid. Please use this link to learn more about the Medicaid policy as defined by the state of Colorado.
